| Please note: WITH THE EXCEPTION OF THE LOC SDI, APPLICATIONS ARE APPROVED ON THE LAST WORKING DAY OF THE MONTH IN WHICH THEY ARE RECEIVED OR TO WHICH PAYMENT RELATES. PROVIDED YOUR EXPERIENCE AND ASSESSMENTS MEET THE REQUIREMENTS, YOU CAN EXPECT TO RECEIVE CONFIRMATION OF THE AWARD APPROXIMATELY TEN WORKING DAYS THEREAFTER. THERE WILL BE A RE-PRINT CHARGE OF £15 IF NON-RECEIPT OF A CERTIFICATE IS ADVISED MORE THAN THREE MONTHS FROM DATE OF AWARD OR FOR LOST CERTIFICATES (AT THE DISCRETION OF THE FACULTY). |
Applications should be accompanied by the Registration and appropriate Subscription fee. All subscription (not registration) fees are apportioned on a quarterly basis.

To ensure that your documentation is received safely, please make certain that the correct postage is paid at the time of dispatch. Our experience is that applications usually require at a least a LARGE stamp. Where there is insufficient postage, the item is not delivered to the Faculty. |
Reduced subscription for Diplomates:
If you do not expect your total gross income from ALL medical activities, for a calendar year (1st January to 31st December), to exceed £20,000, you may be eligible to apply for the reduced subscription rate. An income declaration will need to be signed and submitted with the application. For a copy of the declaration and advice on the reduced fee structure, please contact the Faculty Office 020 7724 5647/5669.
OTHER INFORMATION: The first subscription should be paid by cheque. Subscriptions for subsequent years may be paid by Direct Debit, which will be collected on or about the 1st January annually.
Income tax relief in respect of annual membership subscriptions: The Faculty has been approved by the Board of Inland Revenue under section 201 Income and Corporation Taxes Act 1988 with effect 6th April 1993 under reference SAPPT1644471993JEM dated 30th June 1994.
